Application For Accreditation – Non-Formal

 
Introduction

The service provision of activities as part of non-formal education requires one to ensure that the activity has an educational value and is provided safely, under the supervision of an expert in the field.

As a service provider you are required to fill in the necessary form with the requested information to become an accredited service provider.

It is advisable to view the Terms and Conditions (Click Here to View Terms and Conditions)​ and prepare the following required documents before you start with the application process:

  • ​Police conduct certificate of each adult involved in the activity issued not earlier than 2 months prior to registration.
  • First aid certificate: at least one first-aider must be on the premises at all times during the activity.
  • Verification of adherence to Protection of Minors (Registration) Act for all the adults who are directly involved. Click Here to download form.
  • Risk assessment or letter by a registered Health & Safety Officer of the site/s where the activity will be taking place.
